Fees & Insurance

  • Leah Erickson accepts Cigna/Evernorth insurance. If you choose to use insurance, your co-pay will vary based upon your individual provider and plan. Leah’s private pay rate is $130.00 per 50 minute session.

  • Jilly dos Santos accepts Cigna/Evernorth insurance. If you choose to use insurance, your co-pay will vary based upon your individual provider and plan. Jilly’s private pay rate is $130.00 per 50 minute session.

  • Laura Fonseca accepts Anthem, Blue Cross Blue Shield/Carelon Behavioral Health, Cigna/Evernorth and United Healthcare insurance. If you choose to use insurance, your co-pay will vary based upon your individual provider and plan. She accepts a limited number of clients choosing to utilize insurance. Laura’s private pay rate is $160.00 per 50 minute session.

  • Suzanne Holtshouser accepts Anthem, Blue Cross Blue Shield/Carelon Behavioral Health, Cigna/Evernorth and United Healthcare insurance. If you choose to use insurance, your co-pay will vary based upon your individual provider and plan. Suzanne’s private pay rate is $130 per 50 minutes.

  • Jessie Markowski accepts Aetna, Anthem, Blue Cross Blue Shield/Carelon Behavioral Health, Cigna/Evernorth and United Healthcare insurance. Jessie’s private pay rate is $130.00 per 50 minute session.

Sliding Scale

At Cornerstone Therapy, we recognize that the benefits of therapy should not be limited to those who have the resources to pay the full cost of sessions. As a result, all of our therapists offer a limited amount of sliding scale openings.

Out of Network Reimbursement

If we do not accept your insurance, you may still be eligible for reimbursement through your insurance provider’s out-of-network benefits. Here’s how the process typically works:

  1. Check Your Insurance Coverage
    Contact your insurance provider to ask about your out-of-network benefits. Be sure to inquire about:

    • Reimbursement rates for mental health services.

    • Any deductible that must be met before reimbursement.

    • The percentage of the fee they will reimburse.

  2. Get a Superbill
    Upon request, each month, we will provide you with a detailed receipt, known as a Superbill. This document includes all the necessary information for your insurance company, such as:

    • Date of service.

    • Type of service provided.

    • Provider’s license information.

    • The amount paid.

  3. Submit the Superbill to Your Insurance
    You will need to submit the Superbill to your insurance company. Each provider has its own process, so check with your insurer on how to do this. It’s usually as simple as:

    • Logging into your insurance portal and uploading the document.

    • Mailing the Superbill to the address provided by your insurer.

    • Or using a mobile app provided by your insurance.

  4. Receive Reimbursement
    Once your insurance processes your claim, they will reimburse you directly according to your plan’s out-of-network benefits. This can take several weeks, depending on your provider.

  5. Track Your Claims
    Keep a record of the Superbills you submit and monitor your insurance claims to ensure everything is processed correctly.

Cancellation policy

Clients can cancel or reschedule an appointment anytime, as long as 24 hours notice is provided. If you cancel an appointment within 24 hours notice, or fail to show up, you will be personally charged for the full fee of the appointment regardless of whether you have insurance. If you have a copay, that rate does not apply in this case, because we cannot charge your insurance for a missed session.
